Personalized Treatment Plan
60 Minute In person visit (if your package has an in person visit included otherwise this is a 30 minute visit).
Unlimited 24/7 messaging (customer service available M-F 9:00 am-5:00 pm Est)
Includes (3) 15 Minute Video Visit’s. Video visit are required 1x every 180 days Missed video visits for any reason will be deducted from your available video visits (no longer charged to reschedule if you have visits available). If you do not have any remaining visits available you will be billed current visit pricing. You must cancel or reschedule more than 24 hours in advanced to avoid being charged the full amount
Includes (4) Urgent Care Consults - No Appointment Needed Available during normal business hours only. Complete intake questionnaire only. Provider will Review and create a custom treatment plan based on your condition and medical history. Prescriptions will be sent to your pharmacy on file unless requested otherwise on intake form. THIS IS NOT TO BE USED FOR MENTAL HEALTH CONCERNS. MENTAL HEALTH MUST HAVE VIDEO VISIT.
Includes doctor note/work excuse
Includes (4) Prescription Refills for 3 & 6 month supply of chronic illness medications Refills are also included with all in person and video visits Missed refill appointments for any reason including incomplete intake forms will deduct 1 refill from your available visits. If you do not have any remaining visits available you will be billed current visit pricing. You will use a refill visit to change the prescription or to send to a new pharmacy
